What to do BEFORE your appointment:

  1. All paperwork must be completed and submitted prior to your appointment.  If you haven’t done so yet, click on our forms page to fill everything out.  If you need to upload extra documents, you can do so below or on the forms page. 
  2. Write down your vital signs.  This includes your current weight, your height, your heart rate (count how many heartbeats in 1 minute), and blood pressure (if you have a BP cuff at home).  *Please be honest about your weight since providing the correct medication dose depends on it.
  3. Try to be in quiet room away from distractions.
  4. Write down any questions you want to to ask and have a pen & paper available to take notes.
  5. Have your cell phone available in case there are any problems with the video call.

How to SIGN-IN to your appointment:

  1. Sign on 10-15 minutes prior to your appointment to make sure that everything is working.  
  2. Click on the correct Telehealth link for your Provider.  These links are also in the bar at the top of the page.  
  3. Type in your first & last name and then “Check In”.  
  4. If you can’t hear or see, hover your mouse over the video of your face and click on the 3 dots, then Device Settings.  There is also a Pre-call Test in the bottom Left of the screen to make sure that your settings are correct.  If any other issues, restart your computer.
  5. If you can’t figure it out or cannot get online for some reason, the Provider will attempt to call your cell phone.  

What to do AFTER your appointment:

  1. To schedule a follow up appointment per your Provider’s recommendations, click here
  2. If your Provider suggested any vitamins or supplements, click here.  They may also email you the suggestions.  
  3. If your Provider suggested any specialty tests, click here to read about them and then here to purchase them.
  4. For all questions about your prescriptions and pharmacies, click here
  5. For instructions on insurance reimbursement, click here.  You will be emailed a receipt within 1 business day.
