American Thyroid Institute, LLC (“us”, “we”, or “our”) DBA MyThyroidDoctor operates https://www.MyThyroidDoctor.com (the “Site”). This page informs you of our policies regarding the collection, use and disclosure of Personal Information we receive from users of the Site.  We use your Personal Information only for providing and improving the Site.  By using the Site, you agree to the collection and use of information in accordance with this policy.

Information Collection And Use: While using our Site, we may ask you to provide us with certain personally identifiable information that can be used to contact or identify you, such as in contact or comment forms. Personally identifiable information may include, but is not limited to your name and email address (“Personal Information”).

Log Data: Like many site operators, we may collect information that your browser sends whenever you visit our Site (“Log Data”).  This may include information such as your computer’s Internet Protocol (“IP”) address, browser type, browser version, the pages of our Site that you visit, the time and date of your visit, the time spent on those pages and other statistics.  In addition, we may use third party services such as Google Analytics that collect, monitor and analyze this data for multiple reasons, including but not limited to, marketing and analysis, security monitoring, and improving system performance.  Social media widgets may also track your data as well.

External Links: In order to provide visitors with greater value, we may provide links to various third party websites.  However, even if a third-party affiliation exists between us and that destination site, we exercise no authority over linked sites, each of which maintains independent privacy policy and data collection policies and procedures.  We assume no responsibility or liability for these independent methods or actions.  Similarly, we cannot take responsibility for the privacy initiatives or the content of such websites.  These destination links are provided only for your convenience, and as such, you access them at your own risk.  

E-Commerce and E-Healthcare Transactions: Some of our site features may use applications to collect data to facilitate an e=commerce or e-healthcare transaction.  We will ask your explicit permission to transfer this data to the third party to facilitate this transaction.  We may store this information to verify the transaction to verify the transaction for customer service and accounting reasons and may use this information to help personalize your experience on the Charleston Thyroid Center site.

Communications: We may use your Personal Information to contact you with newsletters, marketing or promotional materials, and other relevant information.

Cookies & Pixels: Cookies and pixels are files with small amount of data, which may include an anonymous unique identifier. You can instruct your browser to refuse all cookies or to indicate when a cookie is being sent. However, if you do not accept cookies, you may not be able to use some portions of our Site.

Changes To This Privacy Policy: This Privacy Policy is effective as of April 5, 2021 and will remain in effect except with respect to any changes in its provisions in the future, which will be in effect immediately after being posted on this page.  We reserve the right to update or change our Privacy Policy at any time and you should check this Privacy Policy periodically. Your continued use of the Service after we post any modifications to the Privacy Policy on this page will constitute your acknowledgment of the modifications and your consent to abide and be bound by the modified Privacy Policy.  If we make any material changes to this Privacy Policy, we will notify you either through the email address you have provided us, or by placing a prominent notice on our website.

How Your Medical Information May Be Used:

  1. TREATMENT: MyThyroidDoctor may use your health information to provide you with medical treatment or services. For example, information obtained by a health care provider, such as a physician, nurse, or other person providing health services to you, will record information in your record that is related to your treatment. This information is necessary for health care providers to determine what treatment you should receive. Health care providers will also record actions taken by them in the course of your treatment and note how you respond to the actions. MyThyroidDoctor may use your health information when referring you to other health care professionals and facilities.
  2. PAYMENT: MyThyroidDoctor may use and disclose your health information to others for the purposes for you to get reimbursed payment for treatment and services that you receive. For example, we will provide you with a superbill and you may decide to submit the superbill to your insurance carrier or a health plan. The information on the bill may contain information that identifies you, your diagnosis, and treatment or supplies used in the course of treatment. MyThyroidDoctor may use your information to contact you about account balances. 
  3. HEALTH CARE OPERATIONS: MyThyroidDoctor may use and disclose health information about you for operational purposes. For example, your health information may be disclosed to members of the medical staff, risk or quality improvement personnel, and others to: Evaluate the performance of our staff; Access the quality of care and outcomes in your cases and similar cases; Learn how to improve our facilities and services; Determine how to continually improve the quality and effectiveness of the health care we provide. 

  4. REQUIRED BY LAW: MyThyroidDoctor may use and disclose information about you as required by law. For example, we may disclose information for the following purposes: For judicial and administrative proceedings pursuant to legal authority; To report information related to victims of abuse, neglect, or domestic violence; To assist law enforcement officials in their law enforcement duties. 

  5. APPOINTMENT REMINDERS: MyThyroidDoctor may contact you to provide appointment reminders or information about treatment plans, medication, test results, other health-related benefits and services that may be of interest to you. When contacts are made via telephone, messages will be left on answering machines with limited information. 

  6. NOTIFICATION: MyThyroidDoctor may use or disclose information to notify or assist in notifying a family member, personal representative, or another person responsible for your care, your location, and general condition.
  7. COMMUNICATION WITH FAMILY: MyThyroidDoctor Providers and staff, exercising their best judgment, may disclose to you family member, other relative, close personal friend, or any other person you identify as allowed to have access to your care, health information relevant to that person’s involvement in your care or payment related to your care.
  8. BUSINESS ASSOCIATES: In some cases, MyThyroidDoctor contracts with business associates to provide services on its behalf. Examples include people who work on or repair our infrastructure (computers, utilities), our electronic health records vendor, financial institutions, etc. MyThyroidDoctor may disclose your health information to such a business associate so that they can perform their respective job functions. To protect your health information, however, MyThyroidDoctor requires the business associate to safeguard your information. 

  9. PUBLIC HEALTH: Your health information may be used or disclosed for public health activities such as assisting public health authorities or other legal authorities to prevent or control disease, injury, disability or for other health oversight activities.
  10. DECENDENTS: Health information may be disclosed to funeral directors or coroners to enable them to carry out their lawful duties. 
  11. ORGAN/TISSUE DONATION: Your health information may be used or disclosed for organ or tissue donation purposes. 

  12. HEALTH AND SAFETY: Your health information may be disclosed to avert a serious threat to the health or safety of you or any other person pursuant to applicable law.
  13. FOOD AND DRUG ADMINISTRATION (FDA): MyThyroidDoctor may disclose to the FDA health information relative to adverse events with respect to food, supplements, product and product defects, or post marketing surveillance information to enable product recalls, repairs, or replacement. 
  14. GOVERNMENT FUNCTIONS: Specialized government functions such as protection of public officials or reporting to various branches of the armed services that may require use or disclosure of your health information. 
  15. WORKERS COMPENSATION: Your health information may be used or disclosed in order to comply with laws and regulations related to Workers Compensation. 
  16. OTHER USES: Other uses and disclosures will be made only with your written authorization and you may revoke the authorization except to the extent MyThyroidDoctor has taken action in the reliance on such. 
  17. YOUR HEALTH INFORMATION RIGHTS: You have the right to: Request a restriction on certain uses and disclosures or your information; however your signature confirms you have read and understood this document and must accompany your history sheet.
