Physicians must hold a medical license in the state that the Patient is physically located to legally establish a doctor-patient relationship and to practice medicine.  We do not yet have any Providers who are licensed in your state.  If you’d be interested in an appointment in the future, please fill out the form below.  We’ll contact you if we become licensed in your state. 

EXCEPTION: If you live in a county that border a state that we’re licensed (e.g.: Rockville, MD directly borders with Virginia and Las Vegas borders with Arizona).  However, legally you will still need to drive across the state border to do all your appointments.  Email us a copy of your ID with current address and we will review this on a case-by-case basis.
